Your Neurological Consultation
During your neurological consultation you will be asked questions with regard to your chief complaint and your present symptoms as well as any past medical history and family history you can provide. The doctor will perform a thorough neurological consultation and examination. If any testing is required, these tests will be ordered and a follow up visit will be arranged to go over the results of these tests.
For your neurological consultation, please bring with you the following:
- A list of current prescriptions and over-the-counter medications with the dose and frequency
- Your insurance card
- Physician referral forms if required by your insurance provider
Pertinent information about your medical and surgical history
- Any previous imaging such as CT or MRI scans and results of any tests you have had.
